Moy Davitts GAA Player Injury Procedure
If any player on the Senior, Junior or Underage football teams incurs an injury during the course of an official competitive game, an official challenge game or an official & supervised team training session, the injured player needs to:
- Obtain a copy of the Claims Form via link below or from the club’s Insurance Officer.
- The injured player must obtain medical certification before any treatment of an injury is received. To this end section E of the claims form must be completed by the attending doctor/dentist.
- The injured player should complete the claims form, in particular sections A, E & F, sign the form and return it, along with all relevant supporting documentation, to the Insurance Officer as soon as possible after initial injury. Section B of form should also be completed if loss in wages > 1week.
- The injured player should keep safe all Receipts received from Doctors/Physios/Scans/Dentists/etc… as these will need to be sent into the Insurance company once treatment of injury is complete.
Notes:
- Each player must see team doctor / your doctor before any treatment can commence.
- Each new injury requires a new form to be completed.
- Each completed claim form must reach Insurance Company within 60days of first injury, otherwise not processed. Therefore it is important to complete the above process immediately upon getting injured to allow for delays.

Exceptions:
If a player is unable to get a Doctor to stamp form or Insurance Officer to sign off on the form at an early stage, then he can still submit Section A & B themselves to the above address inside the 60day limit to keep the claim alive. Thereafter, the remainder of the form can be completed by Doctor, Club & County secretaries and forwarded to above address along with relevant receipts.
Guide to expenses that can be claimed back and limits
- Medical/Dental (excluding below) upto Max €5,000
- Exceptions - Physiotherapy, Osteopathy, Chiropractic, Sports Massage, Acupuncture etc (carried out by member of a professional body, e.g. ISCP), must be medically prescribed and are limited to €200 in total per claim. However medically prescribed post operative treatment is exempt from the limit of €200
- Loss of wages: €0 for wk 1; €200 for each of wks 2-5; €400 for each of weeks 5-52…
- Note: Relates to claims not made thru VHI/Quinn/Vivas
- First €60 of each claim exempt (called ‘Excess’); limits & payouts subject to change.

Working Example:
- Player X gets injured during a training session on the 1st June.
- Player X obtains a copy of the Claims Form, fills in Section A (and B where loss of wages) and brings with him when getting assessed by Doctor/Dentist.
- Player X visits a Doctor/Dentist to assess injury and to certify that he needs prescribed medical, physiotherapy or other treatment. Section E of form is filled in and stamped by the Doctor.
- Player X forwards form with completed sections A, B(if necessary) and E to the Club Insurance Officer.
- Club Insurance Officer signs and forwards to County Secretary.
- County Secretary then approves and forwards to GAA Insurance Administration… Address: GAA Injury Scheme, WILLIS, Grand Mill Quay, Barrow Street, Dublin 4.
- This process needs to be completed within 60days (i.e. posted & delivered by 31st August), otherwise claim will not be processed.
- Receipts as result of treatment for this injury can be forwarded directly by the player to the above address at a later stage.
Example of how a player may receive a refund from scheme:
Player X incurs the following cost during his injury
